We are growing our team!

Lighthouse Community Care, LLC is a small and motivated agency looking to add Case Managers to our team. 
 

We provide our Case Managers equipment, technology, tools, and templates to best support our members and the Case Managers time management. We recognize that policies, protocol, and processes are changing or developing to better services that are provide. We support our Case Managers with paid comprehensive, hands-on, and required trainings at their own pace.  We provide regular supervision for processing, questions, barriers, accomplishments, and training needs. We provide mileage reimbursement, hiring bonuses, health insurance and a total of 21 vacation days to support with self-care.
 

Case Managers will experience a wide range of knowledge of other services within the field. We support Case Managers with developing and sharpening their areas of expertise within the field. Case Managers will have the opportunity to support our members with, for, or on their behalf to meet their needs identified in their Person-Centered Plan/plan of care. Case Managers will have opportunity to listen to our members, hear their stories, and watch their development and accomplishments over time.
