We link you with resources to meet your needs

We complete applications and renewals for MaineCare Waiver funding

We work collaboratively with teams and providers to support you as you navigate through life

We provide hope and action steps to promote your strength and growth

Eligibility

Adult Community Case Management eligibility is determined by the State of Maine Department of Health and Human Services Office of Aging and Disability Services. We support adults age 18+ with a diagnosis of Intellectual Disability and/or Autism.
